From the publisher:

Condition chips are designed to solve the perennial problem of players struggling to remember the specific rules around the various maladies that can be afflicted upon them or that they can inflict on the poor unwitting monsters in the 5th edition of the world's most popular roleplaying game!

The chips have been designed with maximum versatility in mind. So if you know the rules of the conditions you're facing then you can slide it under the character and get a battlefield representation of the maladies you're currently facing!
